WSAVA guideline
WSAVA Global Dental Guidelines
The guideline standardizes periodontal assessment through complementary scoring tools: AVDC periodontal staging (PD0–PD4, Box 5) and mobility grades (M0–M3, Box 4) recorded per tooth, furcation grades I–III for multirooted teeth, and whole-mouth indices — the conscious Oral Health Index (OHI, Gawor, 0–12 with action bands) and the anaesthetized Total Mouth Periodontal Score (TMPS). Findings are charted on the (modified) Triadan numbering system. Probing is circumferential (6 sites/tooth) against normal sulcus depths of 0–3 mm (dog) / 0–1 mm (cat), with bleeding on probing the earliest gingivitis sign, preceding erythema. Staging combines probing with dental radiographs; plaque/calculus timelines (24 h / 3 days / 2 weeks) underpin the homecare logic.
WSAVA Global Dental Guidelines (Niemiec et al. JSAP 2020)
Key findings
- Box 5 — PD staging per tooth (AVDC), verified from official translation: PD0 = clinically healthy, no gingival inflammation; PD1 = gingivitis only, no attachment loss; PD2 = early periodontitis, attachment loss <25% (probing, or radiographic CEJ-to-alveolar-margin : root-length ratio) or furcation stage 1 at most; PD3 = moderate, loss 25–50% or furcation stage 2; PD4 = advanced, loss >50% or furcation stage 3. One patient may carry teeth at different PD stages.
- Box 4 — mobility (M), verified from official translation: M0 physiological ≤0.2 mm; M1 >0.2 to 0.5 mm in any non-longitudinal direction; M2 >0.5 to 1.0 mm; M3 >1.0 mm or any longitudinal mobility.
- Furcation (Figure 114): grade I — probe enters <1/3 root width → cleanable closed (no flap); grade II — >1/3 but not through → flap surgery or extraction; grade III — through-and-through → extraction.
- OHI (Gawor et al. 2006), conscious patient: scores lymph nodes, dental deposits, inflammation status, diet, homecare. Action bands as printed: 0–3 prophylactic recommendations; 4–7 radical change required, surgery likely; 8–12 immediate surgery required.
- TMPS (Harvey et al. 2008): under GA; "allows for a very accurate determination of the patient's periodontal health".
- Modified Triadan: quadrants clockwise from right maxilla — 100s right maxilla, 200s left maxilla, 300s left mandible, 400s right mandible (500–800 deciduous); canines always 04, first molars 09 (e.g., left maxillary fourth premolar = 208).
- Normal probing depth: dog 0–3 mm / cat 0–1 mm (§1, §4); pocket diagnosed at >3 mm (dog) / >0.5 mm (cat) ⚠️ §5 Step 8 prints the feline sulcus as "0-0.5 mm" — internal inconsistency [verify]; record 6 sites/tooth (§4 allows 4–6).
- Bleeding on probing = grade II gingivitis (Figure 9) — first sign of disease, before colour change; gingival inflammation, not calculus load, should trigger professional therapy (calculus is essentially non-pathogenic).
- Timelines: plaque forms within 24 h, calculus within 3 days, gingivitis begins as early as 2 weeks (§1 Key Points).
- Recording: manual symbols — circle (O) missing, hash (#) fractured, cross (X) extracted; free EVDS electronic charting; thiol test strip aids conscious scoring and compliance.
Recommendations
Stage PD per tooth (PD0–PD4, Box 5) combining clinical findings with dental radiographs (AVDC 2019); staging without radiographs is "very inaccurate"
Expert consensus (Dental Guidelines Committee)
Grade mobility (M0–M3) and furcation exposure (I–III) on every multirooted tooth under GA
Expert consensus
Probe and record 6 points circumferentially per tooth, systematically from the first incisor distally; chart with modified Triadan
Expert consensus
Use OHI (Gawor) on conscious patients for the preliminary plan; TMPS under GA for accurate whole-mouth assessment
Expert consensus
Treat bleeding on probing as the earliest sign (grade II gingivitis); judge therapy need by inflammation, not calculus
Expert consensus
Thorough oral examination "MUST be part of every dental procedure", charted contemporaneously in the medical record
Expert consensus
Box 4/5 absent from English capture, verified via official Russian translation (pp. E73–E74); furcation grades from Figure 114 caption [verify Box 3 against PDF]
[verify] flag carried

From the guideline full text (§1 timelines, §4 Oral Examination and Recording — OHI form, TMPS, Triadan, manual/electronic scoring; §5 Step 8 probing/charting; Figures 107–120), cross-checked against pack summary 05. Box 5/Box 4 verified against the official Russian JSAP translation; Box 3 not captured verbatim; the feline sulcus discrepancy (0–1 vs 0–0.5 mm) is printed in the guideline itself.
